Five intake fields patients always get wrong

Small wording changes that cut callbacks and clean up the data arriving at your front desk.

Most bad intake data is not the patient being careless — it is a confusing field. A few predictable culprits cause most of the callbacks: insurance ID formats, date of birth ordering, secondary insurance, pharmacy details, and emergency-contact relationships.

The fixes are small. Show an example next to the insurance ID. Use a date picker instead of a free-text box. Mark secondary insurance as optional so people stop inventing answers. Ask for the pharmacy by name and cross-street, not a guess at the address.

Clean these five up and the verification calls drop noticeably. Digital intake makes the changes easy because the form is no longer frozen on a printed page.
